Nirvana Holistics - Terms and Conditions

Please read these Terms and Conditions ("Terms") carefully before using the services provided by
Nirvana Holistics LLC ("Nirvana Holistics," "Company," "we," "our," or "us"). By scheduling or
receiving services, you ("Client," "you," or "your") agree to be legally bound by these Terms.
1. Nature of Services
Nirvana Holistics is a mobile IV hydration and wellness service. All services are provided on a
self-pay basis. We do not accept or bill health insurance. Clients are financially responsible for all
services received. Our services are designed to promote wellness and support hydration, vitamin
balance, and general health. They are not intended to diagnose, treat, cure, or prevent any disease.
The services provided by Nirvana Holistics do not replace medical advice, primary care, or
emergency care. All clients are encouraged to maintain a relationship with their primary care
provider.
2. Good Faith Exam Requirement
In accordance with Michigan law, all clients must complete a Good Faith Exam (GFE) with a
licensed healthcare provider prior to receiving any IV infusion or injectable therapy. This exam must
be conducted virtually through a designated telehealth link provided by Nirvana Holistics LLC. The
GFE is a separate medical appointment billed directly to the client by the GFE provider. Nirvana
Holistics LLC does not manage or control the fees, scheduling, or scope of care provided during the
GFE.
To ensure timely and efficient completion of your scheduled appointment, it is strongly
recommended that the GFE be completed in advance of the nurse's arrival. If the GFE is delayed or
completed on demand at the time of the appointment, your treatment start time may be postponed.
If adequate treatment time cannot be ensured due to GFE delays, the appointment may need to be
rescheduled at the discretion of the nurse.If the nurse arrives and the appointment must be rescheduled due to an incomplete or pending
GFE, a rescheduling fee of $30 will apply to help cover travel and administrative costs. No IV
hydration or injectable treatments will be administered until confirmation of a valid and approved
GFE is received.
3. Scheduling and Cancellations
Appointments may be scheduled in advance or requested on demand, based on nurse availability.
While we strive to be flexible, the following cancellation policy applies:
- A cancellation fee of $50 will be charged for any scheduled appointment canceled less than 4
hours before the scheduled appointment time.
- No fee is charged for cancellations made more than 4 hours in advance.
- On-demand visits may be rescheduled at nurse discretion and do not carry a cancellation fee
unless a nurse is already en route.
By booking an appointment, you agree to this cancellation policy.
4. Service Window and Arrival Flexibility
Clients agree to allow for a reasonable time buffer surrounding their scheduled appointment to
account for travel distance, traffic delays, weather, or variations in earlier appointments. If a nurse is
delayed due to an extended prior visit or unforeseen circumstance, we will notify you as soon as
possible. For on-demand visits, if a GFE has not yet been completed at the time of nurse arrival, the
wait time to complete the GFE is not guaranteed and may vary depending on provider availability.
Clients should be prepared for this potential delay and understand that treatment may be
rescheduled.
5. Payment Terms
Payment is due at the time of service via accepted electronic payment methods. We do not accept
cash or checks. Prices are subject to change without notice. You will be informed of the cost prior toreceiving any service.
While tipping is not required, it is welcomed and appreciated as a reflection of your satisfaction with
the care provided.
6. Client Representations
By using our services, you confirm that:
- You are 18 years of age or older or have a legal guardian present.
- You will truthfully complete all intake forms and health history questionnaires.
- You understand that wellness and IV therapy services are elective and not a substitute for primary
care or emergency treatment.
- You will follow all post-care instructions and contact your primary healthcare provider for any
ongoing or worsening symptoms.
7. Medical Disclaimer
Nirvana Holistics does not provide diagnostic services or treatment for medical conditions outside
the scope of wellness-based IV hydration and vitamin therapies. Services provided are not intended
to diagnose or treat any condition and are not a substitute for appropriate medical care. Clients must
seek advice from a licensed medical provider for any medical concerns. While many clients may
experience perceived or actual benefits from hydration therapy and vitamin support, outcomes may
vary, and no specific results are guaranteed.
8. Service Refusal
We reserve the right to refuse service to any individual for any reason, including but not limited to:
- Incomplete or failed GFE
- Medical contraindications
- Aggressive or unsafe behavior
- Violation of these Terms9. Limitation of Liability
By receiving services, you acknowledge and agree that Nirvana Holistics LLC, its staff, and affiliates
are not liable for any adverse events, reactions, delays, or complications unless caused by gross
negligence or willful misconduct. You accept full responsibility for your participation in all services,
including known and unknown risks associated with hydration or injection-based therapies.
10. Consent and Agreement
By using our website, scheduling services, or receiving treatment, you agree to be bound by these
Terms. If you do not agree, you must not use our services.

IV Therapy Risks & Disclaimers

While IV hydration and vitamin therapy are generally considered safe when administered by trained
professionals, all medical procedures carry potential risks. At Nirvana Holistics LLC, we prioritize
safety and transparency. Clients should be aware of the following possible risks and side effects
associated with IV and injectable treatments:
- Minor discomfort or pain at the injection/IV site
- Bruising, redness, or swelling at or around the injection site
- Infiltration (IV fluid leaking into surrounding tissue)
- Phlebitis (inflammation of the vein)
- Infection at the injection site- Allergic reaction or hypersensitivity to ingredients (including vitamins or additives)
- Headache, lightheadedness, or dizziness
- Nausea, flushing, or metallic taste
- Changes in blood pressure, heart rate, or blood sugar levels
- Electrolyte imbalances (especially with high-dose fluids or nutrients)
- Air embolism (very rare but serious if air enters the vein)
- Overhydration or fluid overload in susceptible individuals
Although rare, more serious complications can occur, particularly in individuals with underlying
medical conditions. Clients are encouraged to disclose their full medical history, medications, and
allergies during the intake process. Nirvana Holistics LLC reserves the right to postpone or decline
treatment if it is deemed unsafe or medically inappropriate.
By proceeding with IV therapy, the client acknowledges understanding of these potential risks and
agrees to receive treatment voluntarily.
